Description
Anterior crural fascia (deep fascia of the leg) is rendered as a dense connective tissue sleeve encircling the tibia and fibula and tethered to their periosteum, forming firm intermuscular septa that partition the anterior and lateral compartments. Medially, the fascia lies immediately superficial to the subcutaneous border of the tibia, while laterally it bridges toward the fibular shaft over the extensor compartment. Inferiorly, the fascial envelope thickens toward the ankle where it continues into the extensor retinacula, creating fibro-osseous tunnels for extensor tendons as they pass onto the dorsum of the foot. Clinically, this is the fascial boundary that turns post-exertional muscle swelling into pain and ischemia in acute compartment syndrome, with the anterior compartment (tibialis anterior, extensor hallucis longus, extensor digitorum longus) and the deep fibular nerve at risk during tibial fractures or reperfusion after vascular injury. The anterior view also aligns with common surgical planning: fasciotomy incisions are placed to decompress the anterior and lateral compartments while protecting the superficial fibular nerve as it emerges from the lateral compartment in the distal leg.
