Description
Running in the anterior compartment of the leg, the anterior tibial vein accompanies the anterior tibial artery and the deep fibular (peroneal) nerve along the interosseous membrane between tibia and fibula. Proximally it lies deep to tibialis anterior and extensor digitorum longus, then continues distally toward the anterior aspect of the ankle beneath the extensor retinacula. Perforating veins connect it to the superficial venous network of the dorsum of the foot, while venae comitantes flank the artery along most of its course. Deep veins of the leg matter because they are where a large share of clinically significant thrombi form, and the anterior tibial vein is a common blind spot during limited lower-extremity venous duplex examinations that emphasize the popliteal and posterior tibial segments. Calf pain and swelling after trauma, immobilization, or surgery can reflect isolated anterior tibial deep vein thrombosis, and its close relationship to the deep fibular nerve and anterior tibial vessels also frames the anatomy at risk during anterior compartment fasciotomy for acute compartment syndrome.
