Adjacent mucosa remains smooth and gently undulating, with the polyp margins clearly demarcated from the flatter background lining. Small caliber vessels course just deep to the surface. Colonic polyps matter because their gross morphology drives real decisions during colonoscopy, from optical assessment (sessile versus pedunculated contour, surface texture, and color) to selection of cold snare, hot snare, or endoscopic mucosal resection for larger, flat lesions. Adenomatous polyps carry malignant potential along the adenoma to carcinoma sequence, and subtle surface irregularity or focal erythema can correlate with dysplasia or recent contact bleeding. This view also helps explain why incomplete resection and missed sessile serrated lesions in the proximal colon remain key contributors to interval colorectal cancer.
