Description
Posterior cutaway anatomy places the paired lungs laterally within the thoracic cavity, their lobes wrapping around the centrally positioned heart in the middle mediastinum. Superiorly, the trachea bifurcates at the carina into the right and left main bronchi, which branch into lobar and segmental bronchi with distal bronchioles leading to an alveolar bed represented within the pulmonary parenchyma. Medial to each lung, the pericardial silhouette contains the right and left atria superior to the ventricles, with the aorta arching superiorly and the descending thoracic aorta continuing inferiorly along the left side of the vertebral column. The esophagus tracks inferiorly in the posterior mediastinum toward the stomach, seen below the diaphragm in partial overlay. A posterior perspective matters when you need to teach how cardiopulmonary structures relate to the back and paraspinal landmarks, where physical exam findings are actually gathered and where many procedures enter the chest. Bronchopulmonary segments and the course of the main bronchi orient bronchoscopy and help explain dependent consolidations in aspiration pneumonia, which often localize to posterior segments when patients are supine. Age context also fits the common overlap of chronic obstructive pulmonary disease, basilar atelectasis, and cardiac enlargement, conditions that shift mediastinal contours and alter breath and heart sound transmission.
