Description
Running longitudinally along the posterolateral lumbar spine, the iliocostalis lumborum forms the lateral column of the erector spinae (sacrospinalis) complex, positioned lateral to longissimus thoracis and well lateral to the lumbar spinous processes. Inferiorly, its muscular and tendinous fibers arise from the iliac crest and posterior sacrum via the thoracolumbar fascia, then course superiorly to attach to the angles of the lower ribs, placing it immediately deep to the skin and superficial fascia in this “beneath the skin” dissection-style view. The lumbar vertebrae sit medially as the axial landmark, while the iliac crests bracket the muscle’s inferior attachments and the ribs define its superior, lateral insertions. Fibers run obliquely superolateral. Clean landmarks. Clinically, this is the paraspinal muscle belly you retract and protect during posterior lumbar approaches, including midline decompression and paramedian (Wiltse) intervals, where the natural cleavage plane between multifidus medially and longissimus laterally can spare muscle. Iliocostalis lumborum is also a common pain generator in mechanical low back pain and postural overload; focal tenderness and myofascial trigger points often refer pain laterally toward the flank and lower ribs, which can mimic costovertebral or renal patterns if you are not careful. Its segmental innervation from the dorsal rami matters during medial branch blocks and radiofrequency ablation planning, because denervation-related atrophy can alter extensor endurance.
