Description
Centered in the right upper quadrant, the liver occupies the hypochondrium and extends medially across the epigastrium, its inferior margin approaching the right costal arch. The right lobe dominates lateral to the falciform ligament, while the smaller left lobe tapers toward the midline. Along the visceral (inferior) surface, the porta hepatis is indicated as the transverse gateway where the hepatic portal vein and hepatic artery proper enter and where the right and left hepatic ducts (forming the common hepatic duct) exit, in close relationship to the quadrate and caudate lobes. Emphasizing the porta hepatis matters because this is the anatomic bottleneck for inflow, outflow, and biliary drainage. Portal hypertension from cirrhosis, common in older adults, changes hemodynamics upstream and drives collateral formation, while biliary obstruction at or near the hepatic ductal confluence produces a cholestatic pattern of jaundice. Surgeons and interventionalists also orient to this region during hepatoduodenal ligament dissection (Pringle maneuver) and when planning TIPS, biliary stenting, or hilar cholangiocarcinoma workup.
