Description
Emerging from the superior border of the scapula, the inferior belly of the omohyoid courses anterosuperiorly across the lateral neck, then transitions at an intermediate tendon that is tethered by a fascial sling near the clavicle. From there, the superior belly runs superiorly and medially to insert on the body of the hyoid, lying deep to the platysma and superficial cervical fascia and crossing the lower portion of the anterior triangle. Laterally, the sternocleidomastoid forms the dominant oblique contour, while adjacent suprahyoid and infrahyoid strap muscles frame the laryngeal region; superficial facial muscles such as orbicularis oculi and orbicularis oris remain visible over a faintly suggested skull. Because the omohyoid is innervated by the ansa cervicalis (C1 to C3) and sits close to the carotid sheath, this view helps explain why infrahyoid dissection and selective neck dissection demand constant awareness of layered planes and tethering fascia. Surgeons use the omohyoid as a practical landmark for cervical lymph node levels and for orienting to the internal jugular vein during exposure, and its course across the lower neck clarifies how strap muscle tension can influence hyoid depression during swallowing and phonation. The three-quarter left perspective makes the muscle’s oblique trajectory easier to follow than in a flat anterior projection.
