Description
Centered in the anterior neck, the larynx continues inferiorly as the trachea, with the esophagus running posterior to the airway in the prevertebral plane. Flanking this visceral column, the thyroid gland occupies the anterolateral trachea, its right and left lobes connected by an isthmus that typically crosses the 2nd to 4th tracheal rings. Laterally, the carotid sheaths frame the compartment, placing the common carotid artery medially and the internal jugular vein more lateral, with the vagus nerve coursing between them. Superficial to the great vessels, the sternocleidomastoid descends toward the clavicle and manubrium while cervical nerves and superficial lymphatics track across the neck. Anterior neck anatomy rewards precision because millimeters matter in airway and endocrine surgery. During thyroidectomy, the recurrent laryngeal nerve ascends in the tracheoesophageal groove and is vulnerable near the ligament of Berry, while the superior laryngeal nerve runs with the superior thyroid vessels near the upper pole; this illustration’s emphasis on neurovascular relationships supports that mental map. Needle placement for internal jugular venous cannulation and operative corridors for carotid endarterectomy both depend on understanding how the jugular vein overlaps the carotid artery and how the ansa cervicalis and cervical plexus lie on or near the carotid sheath.
