Description
Arising from the femoral nerve and traveling distally along the medial leg, the saphenous nerve is traced to its terminal distribution over the medial malleolus and medial border of the foot in a dorsal oblique view. At the ankle it courses anterior to the medial malleolus in the subcutaneous plane, where it runs close to the great saphenous vein and superficial venous tributaries. Extensor tendons to the hallux and lesser toes cross the dorsum of the foot, while the metatarsals and phalanges provide the bony framework deep to the dorsal interossei and short extensor muscle bellies. Small dorsal cutaneous nerve branches spread distally toward the medial forefoot. Clear landmarks. Clinically, this distal segment is the portion most often targeted in an ankle or adductor canal related regional block and the portion most commonly irritated by trauma, tight footwear, or iatrogenic injury near the medial malleolus. The relationship to the great saphenous vein matters during vein cutdown, varicose vein procedures, and endovenous ablation, where postoperative dysesthesia along the medial foot reflects saphenous neuritis or neuroma. This orientation also helps distinguish saphenous sensory territory from superficial fibular (peroneal) nerve distribution on the dorsum of the foot, a frequent point of confusion in bedside neuroexam teaching. Ideal applications include lower limb anatomy teaching in medical and physiotherapy curricula, clinical skills modules on peripheral nerve blocks, and surgical education for medial ankle incisions and saphenous vein harvesting.
