Description

Centered in an anterior (frontal) orientation, the male heart overlies the superior surface of the diaphragm, with the cardiac apex projecting inferiorly and leftward toward the left hemidiaphragm and its central tendon. The right atrium and right ventricle occupy a more anterior position than the left atrium, while the ascending aorta rises superiorly from the left ventricle and the pulmonary trunk courses anterior to the aorta before bifurcating. Inferior to the pericardial silhouette, the diaphragm forms paired domes, its muscular fibers converging medially into a pale, aponeurotic central tendon immediately deep to the heart. Appreciating this heart-diaphragm interface matters when teaching cardiopulmonary mechanics and when planning procedures that traverse the pericardium. Inspiration drives diaphragmatic descent and changes cardiac position, a practical point during transthoracic echocardiography and when interpreting apparent cardiomegaly on frontal chest radiographs. Phrenic nerve vulnerability is also anchored here: the phrenic nerves descend on the fibrous pericardium toward the diaphragm, and postoperative unilateral diaphragmatic paralysis can follow cardiothoracic surgery or pericardial inflammation, altering respiratory excursion and shifting the cardiac apex.
