Description
Running along the posterior forearm, the extensor indicis arises from the distal third of the ulna and adjacent interosseous membrane, lying deep and ulnar to extensor digitorum as it courses distally toward the wrist. From a lateral perspective, the muscle belly hugs the ulna medially while the radius sits more lateral, framing the interosseous space between them. Distally, its slender tendon passes beneath the extensor retinaculum in the fourth dorsal compartment, traveling alongside the extensor digitorum tendons toward the index finger. The hand skeleton, including the carpals, metacarpals, and phalanges, provides clear distal landmarks for insertion on the extensor expansion of digit II. Independent index finger extension is the teaching point here, and this view makes it easy to separate extensor indicis action from extensor digitorum, which often moves the digits en bloc. Clinically, extensor indicis proprius is a common donor in tendon transfer for restoration of thumb extension after extensor pollicis longus rupture, classically after distal radius fracture or attritional rupture in rheumatoid tenosynovitis. Surgeons also care about its course through the fourth compartment when evaluating dorsal wrist pain and tenosynovitis, since compartment crowding can mimic or coexist with extensor digitorum symptoms. Hand and upper-limb anatomy courses can use this as a clean reference for deep posterior compartment layering, dorsal wrist compartments, and the tendon’s relationship to the ulna, radius, and extensor retinaculum. It also fits operative teaching materials for EIP-to-EPL tendon transfer, dorsal wrist exploration, and rehabilitation guides that explain why index extension may weaken after donor harvest.
