Description
Arising from the anterior division of the internal iliac artery, the middle rectal artery courses anteromedially along the lateral pelvic wall toward the mid and distal rectum, running in close company with the middle rectal veins and pelvic autonomic nerves. From a lateral perspective, the rectum sits posterior to the urinary bladder and superior to the anal canal, while the sigmoid colon continues inferiorly into the rectal ampulla anterior to the sacrum and coccyx. Partial removal of the pelvic bones exposes the relationship of this vessel to the bony pelvis, pelvic floor, and the posterior abdominal wall, with small intestine loops draped more anteriorly and superiorly. Red and blue vascular channels frame the supply and drainage around the hindgut. Variable anatomy matters here. The middle rectal artery is often small, duplicated, or absent, and its rectal contribution overlaps with the superior rectal artery (from the inferior mesenteric) and inferior rectal arteries (from the internal pudendal), a pattern that influences bleeding risk and collateral flow during pelvic surgery. For total mesorectal excision and low anterior resection, lateral pedicles may contain the middle rectal artery adjacent to the inferior hypogastric plexus, where injudicious ligation or traction can contribute to postoperative urinary and sexual dysfunction. A useful landmark.
