Description
Sweeping along the radial (lateral) border of the distal forearm, the abductor pollicis longus muscle belly is emphasized beneath intact skin as it narrows into a tendon that courses toward the thumb. Proximally, its fibers arise from the posterior surfaces of the radius and ulna with contributions from the interosseous membrane, then pass distally and laterally, superficial to the deep extensor layer. At the level of the radial styloid, the tendon runs in the first dorsal extensor compartment, typically accompanied by extensor pollicis brevis, before inserting on the base of the first metacarpal and variably sending slips toward the trapezium. Key landmarks are easy to orient. The radius lies lateral to the ulna, and the thumb rays sit anterior and distal in the hand. This lateral perspective matters because the abductor pollicis longus is a primary driver of thumb abduction at the carpometacarpal joint and a stabilizer during pinch and grasp, so its tendon is a frequent pain generator on the radial side of the wrist. Constriction and synovial thickening in the first dorsal compartment produce de Quervain tenosynovitis, classically reproducing pain with the Finkelstein maneuver and guiding steroid injection or surgical release planning. The course also helps differentiate intersection syndrome, where symptoms occur more proximal at the crossing with extensor carpi radialis longus and brevis.
