Rendered in left lateral perspective, the thoracic cage frames the mediastinum with the costal cartilages and intercostal musculature anteriorly and the thoracic vertebral column posteriorly. Deep to the left ribs, the pericardial sac and left ventricle occupy an anteroinferior position, with the cardiac apex directed inferoanteriorly toward the left hemidiaphragm. Inferiorly, the muscular diaphragm rises in a domed contour, its central tendon blending with the fibrous pericardium so the heart sits on the diaphragmatic base rather than floating freely in the thorax. Along the pericardial surface, the phrenic nerve and pericardiophrenic vessels track vertically, anterior to the pulmonary root and closely applied to the fibrous pericardium. That pericardiophrenic relationship matters in practice. Irritation of the fibrous pericardium or diaphragmatic pleura, as in pericarditis or after cardiothoracic surgery, often refers pain to the shoulder via C3 to C5 through the phrenic nerve, and a lateral view clarifies why. The heart’s diaphragmatic contact also explains why diaphragmatic paralysis, eventration, or high hemidiaphragm from abdominal distension can shift the cardiac silhouette and alter subcostal echocardiographic windows. A key landmark.
