Description
Coursing anteriorly from the trigeminal ganglion, the ophthalmic nerve (CN V1) is shown in lateral profile as it approaches the orbit and enters the cranial cavity exit at the superior orbital fissure. The main trunk sits superior to the maxillary division (V2) and anterior to the petrous temporal region, tracking toward the orbital apex where it typically divides into the lacrimal, frontal, and nasociliary nerves. Proximally, V1 relates to the lateral wall of the cavernous sinus, then runs forward to the annulus of Zinn region at the fissure. Clean, surgical anatomy. Understanding this segment matters because lesions along the lateral cavernous sinus and superior orbital fissure create a predictable pattern of sensory loss in the V1 dermatome, often coupled with ocular motor deficits when CN III, IV, or VI are involved. Herpes zoster ophthalmicus follows the ophthalmic division, and involvement of the nasociliary branch (Hutchinson sign at the tip of the nose) raises concern for corneal disease. This is also the corridor navigated in orbital apex decompression and in approaches to cavernous sinus pathology, where identifying V1 helps orient the surgeon and protects corneal sensation.
