Description
Running along the posterolateral cervical spine, the spinalis cervicis is presented as the medial column of the erector spinae, closely apposed to the cervical spinous processes and deep to the trapezius and splenius musculature in a typical lateral neck profile. Superiorly it approaches the spinous process region of C2 to C4, while inferiorly it blends with the thoracic spinalis near the cervicothoracic junction. Adjacent landmarks in this side view logically include the occipital bone and mandible, the cervical vertebrae with their intervertebral discs, and the posterior elements of the upper thoracic cage, with the scapula and clavicle framing the shoulder girdle anterior to the paraspinal column. Anatomically, this view clarifies a common point of confusion in teaching: spinalis cervicis is not a superficial neck extensor, but a short, segmental extensor that sits medial to longissimus cervicis and contributes to cervical extension and ipsilateral stabilization rather than broad rotary motion. That distinction matters when discussing whiplash-associated disorders and posterior cervical strain, where pain generators often include the deep extensor compartment and the adjacent posterior ligamentous complex rather than the more visible superficial muscles.
