Description
Medial anatomy of the anterior knee is centered on the patellar ligament (patellar tendon), extending inferiorly from the apex of the patella to the tibial tuberosity on the proximal tibia. Superior to this band, the quadriceps tendon continues from the quadriceps femoris to the base of the patella, while the medial patellar retinaculum fans from the vastus medialis and medial patellar margin toward the anteromedial tibia. Deep to the ligament, the infrapatellar (Hoffa) fat pad occupies the anterior interval, and the medial femoral condyle and medial tibial plateau frame the joint line posteriorly. Clear landmarks. A medial view matters because it aligns the patellar ligament with the medial stabilizers that clinicians palpate and surgeons work around, including the medial retinacular complex and the pes anserinus region just distal and medial to the tibial tuberosity. In patellar tendinopathy (jumper’s knee), pain localizes near the inferior pole of the patella, while Osgood Schlatter disease targets the tibial tuberosity apophysis, and this angle helps distinguish tendon thickening from adjacent bursal irritation or retinacular strain.
