Description
Layered beneath the palmar skin of the male thenar region, the transverse head of the thumb adductor complex is presented as a broad, strap-like muscle belly spanning from the palmar surfaces of the second and third metacarpal shafts and bases toward the ulnar side of the first metacarpal and proximal phalanx. Medially, its fibers lie deep to the palmar aponeurosis and superficial flexor tendons, while laterally they approach the first metacarpophalangeal joint, blending with the adductor pollicis insertion and the ulnar sesamoid apparatus. The adjacent bony framework of the hand is readable in proper orientation: carpal bones proximally, metacarpals in the mid-palm, and the thumb’s two phalanges distally, with articular cartilage indicated at the carpometacarpal and metacarpophalangeal joints. Clear landmarks. Pinch strength depends on this plane of muscle. Clinically, the transverse head (functionally aligned with adductor pollicis) sits in the deep thenar compartment where the deep branch of the ulnar nerve courses across the palm, so ulnar neuropathy at Guyon canal or distal forearm often presents with weak key pinch and a positive Froment sign from compensatory flexor pollicis longus recruitment. Surgeons also reference this anatomy during approaches to the first web space, repair of thumb ulnar collateral ligament injuries, and decompression procedures where distinguishing adductor fibers from flexor pollicis brevis and the oblique head reduces the risk of iatrogenic weakness.
