Description
Posteriorly, the quadratus lumborum appears as a deep, rectangular muscle on either side of the lumbar spine, spanning between the iliac crest inferiorly and the 12th rib superiorly. Its medial fibers run adjacent to the lumbar transverse processes, placing it lateral to the spinous processes and deep to the posterior thoracolumbar fascia and overlying paraspinal mass. From a full body view in a male, you would also expect the regional landmarks of the pelvis, lower ribs, and lumbar vertebral column to frame the muscle’s borders and myofiber direction. Bilateral symmetry is easy to compare. Quadratus lumborum anatomy matters when interpreting posterior abdominal wall pain, asymmetric trunk side-bending, or rib related complaints, because the muscle can act on the lumbar spine and can depress and stabilize the 12th rib during forced expiration. Palpation and dry needling discussions often miss its depth and its relationship to the thoracolumbar fascia and erector spinae, yet that relationship explains why symptoms can mimic paraspinal strain. For procedural teaching, this posterior anatomy also provides context for quadratus lumborum plane blocks, where local anesthetic is deposited in fascial planes adjacent to the muscle to cover abdominal wall analgesia.
