Description
Posteriorly, the abdominal aorta tracks inferiorly along the anterior aspect of the lumbar vertebral bodies, centered just left of the midline before reaching the aortic bifurcation at approximately the L4 level. From that bifurcation, the right and left common iliac arteries diverge inferolaterally across the pelvic brim toward the sacroiliac region, continuing as external iliac arteries that pass toward the proximal thigh, where the femoral arteries become apparent near the hip. Paired lumbar arteries course laterally from the aorta across the posterior abdominal wall, while sacral and iliolumbar branches align closely with the sacrum and iliac ala in the pelvis. Bone is your landmark. A posterior perspective matters because it keeps the aorta in register with the lumbar spine and pelvic ring, the same relationships a surgeon navigates during retroperitoneal exposure, anterior lumbar interbody fusion (ALIF), or control of iliac vessels during pelvic trauma. Tortuosity, aneurysmal dilation, and the relationship of the bifurcation to L4 to L5 are not academic details, they influence clamp placement and the risk profile for vascular injury. Collateral pathways through lumbar and iliolumbar arteries also explain persistent pelvic or paraspinal bleeding when the main iliac trunks appear controlled.
