Posterior Perspective of the Dorsal Pancreatic Artery

Description

Arising from the proximal splenic artery near the superior border of the pancreatic body, the dorsal pancreatic artery courses posterior to the pancreas and then turns inferiorly and leftward to supply the body and tail. From a posterior perspective, the vessel is seen running close to the splenic vein on the dorsal surface of the gland, with small pancreatic branches entering the parenchyma along its length. Laterally, the artery approaches the splenic hilum as it contributes to the transverse pancreatic (inferior pancreatic) arcade. Short segments of the duodenum and the head of the pancreas frame the right side of the field. Posterior exposure matters because this is where the pancreatic vascular plane meets the retroperitoneum and where bleeding becomes difficult to control once the gland is mobilized. Surgeons rely on this anatomy during distal pancreatectomy and splenectomy, since the dorsal pancreatic artery can be a dominant supply to the pancreatic body and a significant back-bleeder if divided near its origin. Variants are common, including origin from the common hepatic artery or superior mesenteric artery, and the illustration helps clarify why collateral flow may persist even after splenic artery ligation. Bleeding here is unforgiving.
