Extensor pollicis brevis is presented from a posterior (dorsal) perspective as it courses distally across the dorsolateral wrist toward the thumb. Its tendon lies superficial to the dorsal carpal region deep to the extensor retinaculum, running in the first dorsal compartment alongside abductor pollicis longus, then passing toward the base of the proximal phalanx of the thumb. Radial to the central extensor tendons to the digits, dorsal interossei and lumbricals are suggested deeper within the hand, with dorsal venous channels and smaller vascular branches shown in blue overlying or between fascial planes. This angle matters because it tracks the structures that define the radial border of the anatomical snuffbox and the first extensor compartment, a common site of stenosing tenosynovitis in de Quervain disease.
