Description
Posteriorly, the occipital bone frames the craniovertebral junction, with the lambdoid suture marking the junction between occipital and parietal bones. Inferior to the external occipital region, the cervical vertebrae (C1 to C7) align in the midline, their spinous processes and laminae forming the posterior wall of the vertebral canal, then transition into the upper thoracic vertebrae where rib attachments begin laterally. Between adjacent laminae, the paired ligamenta flava span each interlaminar space, forming an elastic posterior seal that sits deep to the spinous processes and superficial to the epidural space. Clear midline anatomy. Posterior access matters here because the ligamentum flavum is the structure encountered immediately after traversing the interspinous region and lamina during a midline or paramedian approach to the vertebral canal. In epidural anesthesia, the classic loss-of-resistance occurs as the needle passes through the ligamentum flavum into the epidural space; calcification or hypertrophy of this ligament contributes to lumbar spinal stenosis and can complicate both neuraxial needle placement and decompressive laminectomy planning.
