Description
Posteriorly oriented pelvic anatomy frames the injured ischium within the bony ring of a male pelvis. The fracture line traverses the inferior part of the os coxae, involving the ischial tuberosity on the posteroinferior aspect, just inferior to the acetabulum and lateral to the sacrum and coccyx. Superior to the injury, the posterior ilium and the auricular surface sit adjacent to the sacroiliac joint, while the greater sciatic notch and ischial spine contour the posterolateral margin. Pain with sitting is expected. A break through the ischial tuberosity matters because it disrupts the primary weight-bearing contact during seated posture and lies at the common origin of the hamstring muscles, so displacement can mimic or coexist with proximal hamstring avulsion injuries. The nearby course of the pudendal neurovascular bundle around the ischial spine and into the pudendal canal, and the proximity of the sciatic nerve as it exits the greater sciatic foramen, make this region a frequent focus during trauma workup and posterior pelvic fixation planning. Clear posterior bony landmarks help correlate physical exam tenderness with CT findings and guide decisions between nonoperative management and open reduction with internal fixation in unstable pelvic ring injuries. Orthopaedic trauma teams can use this artwork for lectures on pelvic fracture patterns, including isolated ischial fractures versus injuries that extend into the acetabulum or posterior column.
