Description
Posterior sacral anatomy is rendered with the median sacral crest centered between the paired dorsal sacral foramina, the superior articular processes abutting the inferior aspect of L5 at the lumbosacral junction. Laterally, the sacrum meets the right and left ilia at the sacroiliac joints, with the posterior iliac crests framing the superior margin of the pelvic ring. An inferior defect at the sacral hiatus is visible above the coccyx, while a fracture line interrupts the posterior cortical surface and traverses adjacent sacral segments. That posterior corridor matters because it is where sacral fractures threaten the dorsal rami, sacral nerve roots within the sacral canal, and the integrity of the posterior pelvic ring, a common problem in high energy trauma and in fragility fractures of the elderly. Fracture propagation toward the sacral foramina correlates with radicular pain, bowel or bladder dysfunction, and, in severe patterns, cauda equina syndrome. This is a hard area to reduce and stabilize, and the relationship to the sacral hiatus also anchors discussions of caudal epidural access and why displaced fragments can complicate needle passage.
