Description
Posterior neck anatomy is presented from a dorsal perspective in an adult male, with the occipital bone superiorly and the cervical spinous processes descending to the upper thoracic vertebrae. Bilateral trapezius muscles occupy the superficial posterior cervical region, spanning from the midline nuchal area laterally toward the acromial ends of the scapulae. Deep to this plane, the posterior scalene would lie more anterolateral, arising from the posterior tubercles of the transverse processes of C4 to C6 and passing inferiorly toward the external surface of the second rib, just lateral to the cervical vertebrae and superior to the first rib. Scapulae frame the upper thorax on either side. This posterior perspective matters because it clarifies which structures can and cannot be accessed from the back of the neck when addressing lateral cervical pain. Clinicians often localize tenderness to the posterior triangle where the spinal accessory nerve (CN XI) traverses superficially to innervate trapezius, and where scalene-related myofascial pain can mimic cervical radiculopathy or contribute to thoracic outlet symptoms by altering first and second rib mechanics. Clean separation of trapezius from the deeper scalene layer also supports teaching of regional fascial planes and safe injection or dry needling zones. Surface anatomy counts.
