Description
Arising from the posterior cord of the brachial plexus in the axilla, the radial nerve passes posterior to the axillary artery and courses inferiorly along the posterior compartment of the arm. It wraps from medial to lateral across the posterior humerus within the radial (spiral) groove, traveling deep to the lateral head of triceps brachii and between the triceps and brachialis before approaching the lateral intermuscular septum. Proximally, the deltoid overlies the posterior shoulder, while distally the nerve continues toward the lateral epicondyle to enter the forearm where it will divide into a superficial sensory branch and the deep branch (posterior interosseous nerve). Clear relationships. Posterior tracking of the radial nerve matters because this segment is the one injured in midshaft humeral fractures and compressed in radial groove entrapment, producing wrist drop with sensory change over the dorsum of the hand. The illustration also supports surgical orientation during posterior approaches to the humerus and triceps-splitting exposures, where the nerve must be identified and protected as it crosses the bone from medial to lateral. It is also the pathway considered in Saturday night palsy from prolonged compression in the spiral groove.
