Description
Positioned on the dorsal aspect of the ulnar wrist, the dorsal ulnocarpal ligament spans from the distal ulna and ulnar side of the distal radioulnar joint region toward the dorsal carpus, blending with the dorsal radiocarpal capsule. From a posterior perspective, the distal ulna lies proximal and medial to the carpal row, while the triquetrum and lunate sit distal to the ulnar head and articulate with the proximal carpal row. Metacarpals and phalanges extend distally from the carpus, providing clear bony landmarks for orientation. A tight dorsal tether. Dorsal ulnar-sided wrist pain often traces back to the ulnocarpal complex, and this view supports teaching the stabilizing role of dorsal capsuloligamentous fibers during forearm rotation and ulnar deviation when the carpus tends to translate relative to the distal ulna. In arthroscopy and open approaches to the ulnar wrist, the relationship between the distal radioulnar joint capsule, the dorsal ulnocarpal ligament, and the proximal carpal bones matters when evaluating TFCC-associated instability, dorsal capsular tears, or ulnocarpal impaction patterns. Clear dorsal bony anatomy helps correlate surface landmarks with deep stabilizers.
