Description

Seen from a posterior perspective, the male scapula is oriented with the glenoid cavity laterally and the medial border forming the vertebral (medial) margin. The scapular spine runs transversely across the dorsal surface toward the lateral angle, expanding into the acromion at its lateral end where it would articulate with the clavicle at the acromioclavicular joint. A diagonally oriented fracture line crosses the posterior surface, interrupting the continuity of the spine and extending toward the acromial process, with the fracture site emphasized in red. Clear bony landmarks remain readable, including the supraspinous and infraspinous fossae divided by the spine. Posterior scapular fractures carry specific teaching value because they sit at the junction of the shoulder girdle’s strut system and the rotator cuff attachments. Disruption of the spine and acromion can alter the origin of the deltoid and trapezius and may compromise the subacromial space, a practical concern in trauma cases where acromial fractures accompany scapular body injuries after high-energy impact.
