Description
Posteriorly, the proximal calf is shown with the plantaris muscle belly as a small, fusiform slip lying deep to the lateral head of gastrocnemius and superficial to soleus, just inferior to the popliteal fossa. From that short belly, the long, slender plantaris tendon descends inferomedially between gastrocnemius and soleus, tracking toward the medial aspect of the calcaneal (Achilles) tendon. Overlying integument and superficial fascia are rendered translucently to keep the plantaris legible without removing the surrounding posterior compartment context. Small but telling. Plantaris anatomy matters because its tendon frequently confuses clinical examination and imaging of the calf, and its rupture is a classic mimic of deep vein thrombosis in the setting of acute calf pain and swelling (the so-called tennis leg when combined with medial gastrocnemius strain). Surgeons also encounter plantaris as an accessory structure during posterior approaches to the ankle and hindfoot, and the tendon is sometimes harvested for grafting or implicated as a contributor to medial Achilles tendinopathy when it courses close to the calcaneal tendon. This posterior, near-surface presentation supports side-by-side teaching with ultrasound and MRI, where the plantaris tendon can appear as a fine linear structure adjacent to the Achilles and be mistaken for a split tear.
