Description
Positioned along the posteromedial ankle, the tendon sheath of flexor digitorum longus tracks inferiorly behind the medial malleolus within the tarsal tunnel, deep to the flexor retinaculum and just anterior to the calcaneal (Achilles) tendon. The distal tibia and fibula form the ankle mortise superiorly, with the talus seated centrally; portions of the calcaneus are typically visible inferiorly. Laterally, the fibula relates to the talus via the lateral ligament complex, while medially the flexor digitorum longus tendon and its synovial covering sit adjacent to the posterior tibial tendon and flexor hallucis longus tendon in their characteristic compartmental order. Posterior ankle views that include the malleolar sulci are where tendon pathology stops being abstract. Flexor digitorum longus tenosynovitis and stenosing irritation under the flexor retinaculum can mimic tibialis posterior dysfunction, and the confined space of the tarsal tunnel also explains why swelling or anatomic variants may contribute to tibial nerve symptoms. Surgical decompression and endoscopic hindfoot procedures rely on this relationship: the tendon sheath marks a safe, teachable boundary between the medial neurovascular bundle and the deeper flexor hallucis longus groove along the posterior talus.
