Description
Sectioned through a quarter of the testis, the cut face exposes testicular lobules packed with seminiferous tubules beneath the dense tunica albuginea, while the posterior border anchors the epididymis with its head superior, body posterolateral, and tail inferior. From the tail, the ductus deferens (vas deferens) emerges as a thick-walled, muscular tube and turns superiorly toward the spermatic cord, traveling medial to the pampiniform venous plexus and alongside the testicular artery and accompanying autonomic nerves. Superficial coverings logically include scrotal skin and dartos fascia, with cremasteric muscle fibers and fascia forming the more proximal coverings around the cord. A clear continuity. That junction between epididymal tail and ductus deferens matters in the operating room and in clinic. Vasectomy targets the ductus deferens in the spermatic cord, but a solid grasp of its relationship to the epididymis and pampiniform plexus helps avoid hematoma and reduces the risk of iatrogenic injury during cord exploration. This is also the anatomic substrate for post-vasectomy pain and for obstructive azoospermia, where sperm granuloma can form near the transection site and backpressure can distend the epididymal tubules.
