Description
Posterior sacral foramina punctuate the dorsal surface of the sacrum as four paired openings aligned in a vertical series on either side of the median sacral crest. Each foramen sits lateral to the fused spinous processes and medial to the intermediate and lateral sacral crests, with the superior foramina positioned just inferior to the base of the sacrum and the inferior foramina approaching the apex above the coccygeal articulation. The dorsal surface contours and ridges define the midline clearly. Surrounding bone landmarks frame each “hole” in the posterior pelvis. Dorsal rami of the sacral spinal nerves traverse these posterior sacral foramina after the ventral rami exit anteriorly, so accurate appreciation of their location supports teaching of sacral dermatomes, posterior pelvic pain referral patterns, and entrapment or irritation in sacral fractures.
