Description
Uterine sagittal section anatomy is centered on the endometrial cavity, cervix, and fundus, with the myometrium forming the thick muscular wall between serosa and endometrium. A focal adenomyoma appears as a rounded, dark intramural lesion embedded within the myometrium, producing asymmetric, uneven thickening rather than a smooth uterine contour. The lesion sits deep to the endometrium and can blur the expected endometrial–myometrial interface (junctional zone), a relationship best appreciated in this fixed midline cut. Orientation landmarks include the uterine body superiorly and the cervical canal inferiorly. One focal mass dominates the wall. Focal adenomyoma matters because it sits on the spectrum of adenomyosis, where ectopic endometrial glands and stroma within the myometrium drive pain and bleeding, and it can be mistaken for an intramural leiomyoma on ultrasound when margins are ill defined. The sagittal plane is the workhorse for correlating symptoms with site, letting you relate the lesion’s depth to the endometrial cavity, assess distortion of the cavity that may contribute to subfertility or recurrent implantation failure, and consider why hysteroscopic access may be limited despite a predominantly intramural process. The darker, rounded focus against surrounding myometrium supports teaching of key distinctions: adenomyoma tends to show ill circumscription and wall thickening, whereas fibroids more often create a discrete, whorled mass with a pseudocapsule and a different surgical feel.
