Description
Arising from the lumbar plexus, the lateral femoral cutaneous nerve (lateral cutaneous nerve of the thigh) descends inferolaterally across the iliacus toward the anterior superior iliac spine, then passes deep to, or through, the inguinal ligament to enter the proximal anterior thigh. From there it courses distal and lateral over the anterolateral aspect of the femur, superficial to the fascia lata, where it divides into anterior and posterior branches supplying skin of the lateral thigh. Bony landmarks include the iliac crest, anterior superior iliac spine, acetabulum, and proximal femur with the femoral head seated medially in the hip joint. A clean anterior oblique angle makes the nerve’s relationship to the pelvis and inguinal region easy to read. Clinical relevance sits at the inguinal ligament. Entrapment of the lateral femoral cutaneous nerve at the level of the anterior superior iliac spine produces meralgia paresthetica, classically burning dysesthesia and numbness over the anterolateral thigh without motor deficit, often provoked by tight belts, obesity, pregnancy, or prolonged hip flexion. Surgeons also track this nerve during anterior approaches to the hip and when placing pelvic pins, because iatrogenic injury can occur where the nerve crosses the iliacus and exits near the inguinal ligament. Purely sensory. That matters.
