Description
Centered in a frontal schematic of the female pelvis, the uterus is opened to reveal the endometrium (uterine lining) and surrounding myometrium, with a submucosal uterine fibroid (leiomyoma, myoma) projecting from the inner wall into the endometrial cavity. The mass sits immediately deep to the endometrium and remains contained within the uterine body, superior to the cervix and medial to the uterine cornua. Fallopian tubes extend laterally from the fundus toward the ovaries, which lie lateral and slightly inferior to the tubal fimbrial ends, while the cervix forms the narrow inferior segment continuous with the uterine canal. Submucosal fibroids matter because their proximity to the endometrium drives symptoms out of proportion to size, classically heavy menstrual bleeding and intermenstrual bleeding from increased surface area and impaired uterine contractility. Fertility impact is common: a lesion that distorts the cavity can interfere with implantation and is linked to recurrent pregnancy loss, so FIGO type 0 to 2 submucosal myomas receive different counseling than intramural or subserosal tumors. Hysteroscopic myomectomy targets this anatomic plane, and this view clarifies why surgeons assess depth of myometrial extension, proximity to the tubal ostia, and the cervical canal as an access route. A common source of anemia.
