Description
Seen from a superior perspective, the left and right anterior cerebral arteries course anteromedially from the region of the circle of Willis toward the interhemispheric fissure, then curve superiorly to run along the dorsal surface of the corpus callosum as the pericallosal arteries. Callosomarginal branches track parallel and slightly lateral to the pericallosal trunk along the cingulate sulcus, while cortical branches extend over the medial aspects of the superior frontal gyrus and toward the paracentral lobule. Gyri and sulci on the convexity frame the vascular tree. Midline anatomy dominates the field. This top down view matters because the anterior cerebral artery is the dominant supplier of the medial frontal and parietal cortex, including leg motor and sensory representations around the paracentral lobule. Infarction in the A2 to A4 segments often presents with contralateral lower extremity weakness, abulia, and urinary incontinence, and subarachnoid hemorrhage from an anterior communicating artery aneurysm commonly threatens this territory while also producing chiasmal compression patterns. Neurosurgeons and interventionalists also rely on the pericallosal and callosomarginal course as midline landmarks when planning interhemispheric approaches and when interpreting CTA or MRA projections that can obscure A1 versus A2 anatomy.
