Description

Cranial nerve pathways are rendered on the internal surface of the skull base, with the foramen magnum centered and the clivus sloping anterosuperiorly toward the dorsum sellae. From the pontomedullary junction, the facial nerve (CN VII) and nervus intermedius (intermediate nerve of Wrisberg) course anterolaterally toward the internal acoustic meatus on the petrous temporal bone, positioned with CN VIII closely lateral and posterior in the same canal. More anteriorly, the trigeminal nerve approaches Meckel’s cave, while lower cranial nerves track toward the jugular foramen and hypoglossal canal in expected posterolateral relationship to the occipital condyles. Bony foramina act as fixed landmarks. Separating the nervus intermedius from the motor root of CN VII matters because this small component carries taste fibers from the anterior two thirds of the tongue via the chorda tympani and parasympathetic fibers to the lacrimal, submandibular, and sublingual glands, a distribution that explains selective symptoms in facial nerve disorders. A surgeon working through a translabyrinthine or retrosigmoid approach to the internal auditory canal must respect the tight packing of CN VII, nervus intermedius, and CN VIII to limit postoperative dysgeusia, dry eye, and reduced salivation after vestibular schwannoma resection.
