Description
Pars interarticularis of a thoracic vertebra occupies the bony bridge between the superior and inferior articular processes, immediately posterior to the pedicle and continuous with the lamina of the vertebral arch. From a superior perspective, the posterior elements frame the vertebral foramen, with the pedicles forming the anterolateral walls and the laminae converging posteriorly toward the base of the spinous process. Lateral to the pars, the transverse process projects outward, while the superior articular facets sit posteromedially, oriented to accommodate thoracic rotation. Small anatomy, big consequences. Stress injury or fracture through the pars is the substrate of spondylolysis and can progress to segmental instability, a concept often taught using lumbar examples but directly applicable when evaluating thoracic pain in athletes, trauma patients, or postoperative spines. For spine surgeons, the pars-pedicle-lamina junction is also the corridor that determines safe trajectories for pedicle screw placement and helps explain breaches that threaten the spinal canal or adjacent costovertebral structures. The fixed superior viewpoint makes facet orientation, the pedicle margins, and the pars as the narrow isthmus of bone easy to parse without distraction from vertebral body detail.
