Description
Centered on the midface, erythema concentrates over the bilateral malar eminences and extends medially across the nasal dorsum and alae, creating a classic centrofacial pattern. Fine telangiectasia can be appreciated superficially within the inflamed skin, running in small arborizing branches that are most apparent over the cheeks and lateral aspects of the nose. Mild edema thickens the soft tissue contours of the cheeks and perinasal region, while the periorbital skin and vermilion borders remain comparatively spared. Pores and fine rhytids are rendered at the surface, reinforcing the clinical texture of inflamed facial skin. Acute rosacea flares matter because the distribution and surface vascular change help separate rosacea from look-alikes: acne vulgaris (comedones), seborrheic dermatitis (greasy scale in the nasolabial folds), allergic contact dermatitis (more diffuse edema and pruritic plaques), and the malar rash of systemic lupus erythematosus (often sparing the nasolabial folds). Facial flushing and fixed erythema are often triggered by heat, alcohol, spicy foods, or emotional stress, and a sudden worsening can follow topical steroid exposure (steroid rosacea) or irritant overuse. Severe or recurrent inflammation over the nasal skin also sets the stage for phymatous change (rhinophyma) in predisposed patients. Look for ocular symptoms. Dermatology and primary care educators can use this rendering to teach recognition of the centrofacial erythema and telangiectasia that define erythematotelangiectatic rosacea and to frame differential diagnosis in case-based learning. It also suits patient-facing materials discussing trigger avoidance, barrier-friendly skin care, and when to consider topical metronidazole, azelaic acid, ivermectin, or vascular laser for persistent telangiectasia.
