Anterior Perspective of a Fractured Ilium

Description

Anteriorly, the male bony pelvis is presented with emphasis on the right ilium, where a fracture line traverses the medial (internal) surface of the iliac fossa beneath the iliac crest. The anterior superior iliac spine and anterior inferior iliac spine sit superior and lateral to the acetabulum, while the arcuate line courses inferomedially toward the pubic body and the pelvic brim. Medially, the fractured ilium approaches the sacroiliac region, and inferiorly the pubis and ischium frame the obturator foramen, with both femoral heads seated in the acetabula to define the hip joints. Iliac fractures rarely matter in isolation. A break across the iliac fossa raises immediate concern for pelvic ring instability, associated sacroiliac joint injury, and hemorrhage from cancellous bone and nearby branches of the internal iliac system, so this view helps you judge whether the lesion behaves like a stable iliac wing fracture or part of a larger APC or lateral compression pattern. Surgical planning also hinges on these anterior landmarks because the ilioinguinal and modified Stoppa approaches use the ASIS, pelvic brim, and quadrilateral surface orientation to reach the inner table for reduction and plating.
