Description
Arising from the inferior pole of the thyroid gland, the inferior thyroid vein descends in the anterior neck, coursing inferiorly toward the brachiocephalic vein at the thoracic inlet. It lies anterior to the trachea and deep to the infrahyoid strap muscles, with the sternocleidomastoid positioned more laterally as a superficial landmark. Medially, the venous channel tracks close to the midline, while the carotid sheath structures (common carotid artery and internal jugular vein) sit posterolateral to the thyroid lobe. For endocrine and anterior cervical surgery, this vessel is the venous structure that surprises people. The inferior thyroid veins can be multiple and thin walled, and they cross the operative field during thyroidectomy and parathyroid exploration, where traction on the gland can tear a tributary and produce brisk low pressure bleeding that obscures the recurrent laryngeal nerve and inferior parathyroid region. An anterior perspective clarifies how the vein approaches the brachiocephalic confluence and why the thoracic inlet is a frequent site for difficult hemostasis.
