Description
Frontal anatomy in an obese adult male is presented with the thoracic viscera superior to the abdominal contents, framed by a thickened subcutaneous adipose layer over the chest and anterior abdominal wall. Bilateral lungs occupy the pleural cavities on either side of the mediastinum, with the heart suggested centrally and slightly left of midline, deep to the sternum. Inferior to the diaphragm, the liver sits predominantly in the right upper quadrant, while the stomach lies in the left upper quadrant with its greater curvature directed inferiorly; coils of small intestine fill the central abdomen and the large intestine outlines the periphery. Clear midline orientation supports left right comparisons. Obesity alters surface landmarks and the working distance to viscera, so an anterior perspective like this helps explain why physical examination findings can be blunted and why anterior approaches often require longer instrument reach. The relationship between a fatty anterior abdominal wall and the underlying bowel also frames operative and procedural risk, including limited workspace during laparoscopic entry and a higher likelihood of ventral and umbilical hernia formation at sites of fascial weakness. Diaphragm position and reduced thoracic compliance are easier to conceptualize when the lungs, heart, and upper abdominal organs are shown together. It matters.
