Description
Running along the anterolateral aspect of the knee, the lateral patellotibial ligament spans from the lateral margin of the patella toward the proximal tibia, approaching the region just lateral to the tibial tuberosity. Superiorly, the distal femur forms the trochlear surface behind the patella, while inferiorly the tibial plateau transitions into the anterior tibial cortex. Lateral to the joint line, the fibular collateral ligament is positioned as a cordlike band descending toward the fibular head, providing a useful landmark for orienting the lateral retinacular and capsular tissues. Orientation is clear. Attention to the lateral patellotibial ligament matters when teaching the layered anatomy of the extensor mechanism and lateral retinaculum, where capsular thickening, iliotibial tract expansions, and patellofemoral supporting ligaments can be conflated in simplified diagrams. This anterior perspective helps clarify the ligament’s relationship to the patella and tibial tuberosity region, a common reference area in lateral retinacular release planning and in discussions of patellar maltracking and lateral patellar tilt. It also supports interpretation of anterior knee pain syndromes where retinacular tightness and altered patellar glide are examined clinically.
