Running anteroinferior to superoposterior on the external laryngeal framework, the oblique part of the cricothyroid muscle arises from the anterolateral arch of the cricoid cartilage and ascends to the inferior margin and inferior horn (cornu) of the thyroid cartilage. Its belly sits lateral to the anterior midline of the larynx, flanking the laryngeal prominence of the thyroid cartilage, with the hyoid bone positioned superiorly and the first tracheal rings continuing inferiorly below the cricoid. From this anterior perspective, the cricothyroid spans the cricothyroid interval, with the thyroid cartilage anterior to the laryngeal cavity and the cricoid forming a complete ring at the base. Fiber direction is the story. Cricothyroid contraction tilts the thyroid cartilage anteroinferiorly toward the cricoid at the cricothyroid joints, lengthening and tensing the vocal folds and raising fundamental frequency, a point that students often confuse with the actions of the thyroarytenoid (vocalis) muscle. Clinically, this surface anatomy matters during cricothyrotomy, where the cricothyroid membrane lies immediately deep to the muscle and adjacent to the midline, and where staying strictly midline reduces risk to the cricothyroid muscle and the nearby cricothyroid branch of the superior thyroid artery. The same relationship helps explain why injury to the external branch of the superior laryngeal nerve produces loss of high pitch due to cricothyroid paresis. Otolaryngology teaching sets, speech and hearing science modules, and anesthesia airway manuals can all use this view to orient learners to the palpable thyroid and cricoid landmarks before correlating with endoscopic laryngeal function.
