Appendicular Vein, Anterior View

Description

Arising from the mesoappendix along the medial border of the cecum, the appendicular vein courses anteriorly to parallel the appendicular artery as it approaches the vermiform appendix. Portions of the terminal ileum and cecum frame the base of the appendix in the right lower quadrant, with the ileocecal junction positioned superior and medial to the appendiceal origin. Small tributaries from the appendiceal wall converge toward the mesoappendix and pass proximally toward the ileocolic venous system, while adjacent lymphatic channels and nodes track the same pedicle. For appendectomy, this is the vessel you control. An anterior perspective clarifies how the appendicular vein lies in the mesoappendix with the artery and accompanying nerves, a relationship that matters when dividing the mesoappendix during open surgery through a McBurney (gridiron) incision or when sealing the pedicle laparoscopically. Venous oozing from an incompletely ligated appendicular vein can obscure the field early, and thrombophlebitis in the appendiceal drainage may contribute to pylephlebitis of the portal venous system in complicated appendicitis.
