Description
Layered beneath intact skin, the anterior forearm is rendered with the superficial fascia removed enough to reveal the flexor mass, with the flexor digitorum superficialis (FDS) occupying an intermediate plane between the most superficial flexors and the deeper flexor digitorum profundus. Laterally, the brachioradialis stands out along the radial border of the forearm as it runs from the lateral supracondylar ridge toward the distal radius, while medially the flexor carpi ulnaris tracks toward the pisiform. Proximally, the biceps brachii and its tendon sit anterior to the elbow, and distal white tendons course toward the palm and into the fingers, aligning with the FDS slips to the middle phalanges. Fiber direction is explicit. So is tendon trajectory. For teaching surface and surgical anatomy, this anterior, palm-forward orientation clarifies why the FDS can be difficult to isolate on palpation despite its size, since its muscle belly lies deep to palmaris longus and parts of flexor carpi radialis, yet its four tendons become clinically conspicuous as they pass through the carpal tunnel with the median nerve. That relationship matters in carpal tunnel syndrome, in flexor tenosynovitis, and when planning or interpreting the standard FDS function test for individual digits, where the profundus is held in extension to assess independent PIP flexion. Tendon lacerations in the distal forearm and wrist (flexor zone V) also hinge on understanding which tendons you are repairing, and where.
