Description
Infraorbital groove (sulcus infraorbitalis) courses anteromedially across the orbital surface of the maxilla, starting near the inferior orbital fissure and continuing forward to become the infraorbital canal. From this anterior viewpoint, the groove sits on the orbital floor, inferior to the orbital rim and superior to the roof of the maxillary sinus, with the zygomatic bone positioned laterally and the lacrimal and ethmoid contributions to the medial orbital wall lying medial to the furrow. The infraorbital foramen is anticipated just inferior to the orbital margin on the facial surface of the maxilla. Bone relief is the message. That sulcus matters because it guides the infraorbital nerve and vessels, and its shallow bony coverage makes them vulnerable in orbital floor (blowout) fractures, where comminution along the groove and canal correlates with infraorbital hypoesthesia of the upper lip, lateral nose, and lower eyelid. In midface trauma and Le Fort fracture patterns, the fixed relationship between the infraorbital groove, the infraorbital rim, and the zygomaticomaxillary region helps localize fracture propagation and plan plate placement while avoiding iatrogenic injury. Direct lighting over the orbital surface clarifies the change in contour where the groove transitions into the canal, a frequent point of confusion when correlating anatomy with CT in the axial and coronal planes.
