Description

Framed from the anterior thorax, the right and left lungs flank the mediastinum while the heart sits anterior to the diaphragm and slightly left of midline, with the right atrium and right ventricle forming most of the visible cardiac silhouette. Superiorly, the great vessels emerge from the base of the heart, and inferiorly the pericardium approaches the fibrous central tendon of the diaphragm near the cardiophrenic angles. The diaphragmatic (basal) surfaces of both lungs sweep inferiorly to meet the domed hemidiaphragms, with the right dome typically higher because of the liver. Orientation is unmistakable. Attention to the diaphragmatic lung surfaces makes this plate useful when teaching mechanics of respiration and how thoracoabdominal pressure changes affect venous return and cardiac filling. The close relationship of pericardium to diaphragm explains why subdiaphragmatic irritation can refer pain to the shoulder via the phrenic nerve (C3 to C5), and why diaphragmatic paralysis alters basal ventilation patterns and can elevate a hemidiaphragm on imaging. Clinicians also read the cardiophrenic and costophrenic recesses for pleural effusion or pericardial fat pads, so the spatial anatomy here maps cleanly onto a frontal chest radiograph.
