Description
Across the distal anterior forearm, the pronator quadratus forms a deep, rectangular muscular sheet spanning from the distal quarter of the ulna medially to the distal radius laterally, immediately proximal to the radiocarpal joint. Superficial skin is retained for orientation, while the radius on the thumb side and the ulna on the little finger side anchor the muscle’s transverse fibers. Distally, the carpal bones and the bases of the metacarpals define the wrist and hand skeleton in anatomical position, with the pronator quadratus sitting anterior to the distal radioulnar joint (DRUJ) and radiocarpal articulation. Bone landmarks, including the olecranon at the elbow and the carpal rows at the wrist, frame the muscle’s location along the forearm axis. Clinical relevance concentrates at the distal radius and DRUJ. Traumatic distal radius fractures commonly injure or strip the pronator quadratus, and on lateral wrist radiographs its swelling can produce the classic pronator quadratus fat pad sign, a subtle cue when cortical disruption is minimal. Surgeons also encounter it directly in the volar Henry approach for open reduction and internal fixation, where elevating the pronator quadratus exposes the distal radius while protecting the flexor tendons and providing a tissue layer for plate coverage on closure. Small muscle. Big landmark.
